Waist-to-Height Ratio
Waist-to-Height Ratio (WHtR) is your waist circumference expressed as a percentage of your height. It is widely considered a better predictor of cardiovascular and metabolic risk than BMI because it specifically measures central (abdominal) fat — the most metabolically active and dangerous type. The rule of thumb: 'keep your waist to less than half your height' maps to a WHtR below 50%, which is associated with significantly lower health risk.

What a high reading means
A high Waist-to-Height Ratio indicates excess abdominal fat relative to your frame. Visceral fat (the fat around internal organs) is metabolically active — it produces inflammatory cytokines, disrupts insulin signalling, raises blood pressure, and independently predicts heart disease, type 2 diabetes, and early mortality. This is often called 'metabolic obesity' — people with normal BMI but high WHtR still carry significant risk.
What a low reading means
A very low Waist-to-Height Ratio (below 43%) may indicate underweight or very low body fat. While this can be healthy in lean, muscular individuals, very low values alongside fatigue or unintentional weight loss warrant medical evaluation.

Why is WHtR better than BMI?
BMI doesn't tell you where your fat is stored. Abdominal fat is far more harmful metabolically than fat elsewhere. Someone with normal BMI but high WHtR ('metabolically obese normal weight') has significantly higher cardiovascular and diabetes risk than someone with high BMI but low WHtR.
What is the ideal waist-to-height ratio?
Below 0.5 (50%) is the widely recommended cutoff — your waist should be less than half your height. Below 0.43 (43%) is optimal. Above 0.58 (58%) is high risk. These thresholds apply across ethnicities but risk begins at lower values in Asian populations.
